Video Production Preliminary Request Form

Please complete and submit the form below to provide preliminary information about your event and to request video services. This form only informs the Mass Communications program about your event and does not confirm reservation. The division head will verify the availability of the students and assess the type of supplies/equipment are needed to fulfill the video request. Upon approval, the video production faculty will assign a crew and notify the contact person to discuss the logistics.
